  How do I.....

 ...Make an appointment

All of the doctors work on a part-time basis, so it is advisable to check their days in attendance under Doctors. We try to keep a balance of appointments available by having appointments on the day, within 48hrs and up two weeks ahead.  The appointment system is updated on a weekly basis and the number of pre-bookable appointments are determined by doctor availability.  Appointments are available during the following times:-

Monday    8.45am - 12.15pm,  2.30pm - 5.30pm

Tuesday 10.15am - 12.15pm & 2.30pm-6.00pm

Wednesday, Thursday & Friday  8.45am - 12.15pm  & 2.15pm - 5.15pm

Telephone Calls - When making an appointment you may be asked if a telephone consultation with the doctor or practice nurse is appropriate.  Some patients find this a useful addition to our services.  It is very important that we have your correct contact details.  If your request is for a telephone consultation with a particular doctor or the practice nurse you may leave a message and they will return your call when they can.  If your call is urgent you will be advised that the doctor on call for that day will return your call.  Please note that all calls are monitored and recorded for training purposes.

....Register as a new patient

You will have be asked by receptionist to complete a questionnaire and have a "registration appointment" with the practice nurse. This will give us some background information, until your records arrive, on past medical problems and lifestyle. you may prefer to discuss parts of the questionnaire personally with the nurse. If you are on regular medication you will need an appointment with a doctor to ensure that you will be able to request further prescriptions.

Health Visitors can be contacted by telephoning 791083 or a message can be left with reception. The Health Visitors will return calls when they are next in surgery.

Home Visits - If you are too ill to attend the surgery you may request a home visit. Whenever possible please telephone the surgery before 10.00am. You will be asked information about your illness so that "visits" can be put in order of priority. If your request is urgent you will be seen by the doctor "on call".  It is not always possible for the doctor of your choice to visit you on a particular time or on a particular day.

Inform us immediately of same. simply fill out a form at reception.

  ...Obtain a sick note

You can self certificate for the first week of your illness. Your employer is obliged to accept this certification. If your employer is not wiling to do so he/she may contact the surgery for a certificate (with your permission of course). There is a fee for this service.

  ...Make a complaint

Please speak to the Practice Manager, the reception staff will provide you with a leaflet explaining the procedure on request.
