You may telephone between 1.30pm and 4.30pm or call in and put the repeat prescription request in the box provided. These prescriptions will have been approved previously by your usual doctor. Alternatively you can write enclosing a stamped envelope or e-mail your request to prescription.request@gp-k83077.nhs.uk You must include the exact details from your repeat prescription form and a daytime telephone number where you can be contacted if necessary.

PLEASE ALLOW TWO WORKING DAYS to process your request and FIVE WORKING DAYS if using the postal system.  If you have not seen the doctor for some time you will be asked to make an appointment before any further medication is issued.
