Private letters, reports and forms

Some services provided are not covered under our contract with the NHS and are therefore chargeable. Examples include the following:

• Medicals for pre-employment, sports and driving requirements (HGV, PSV etc.)
• Insurance claim forms
• Prescriptions for taking medication abroad
• Private sick notes
• Vaccination certificates
• Private letters
• Reports

The fees charged are based on the British Medical Association (BMA) suggested scales and our reception staff will be happy to advise you about them along with appointment availability.

Please note that such requests take between 10 to 21 days to produce dependent on the type of request received.

Private referrals are treated on the same timescale as an NHS referral, they do not take Priority.  The following information is required before the referral can be done:

• The Consultant's name
• The Hospital details
• Provide the Surgery with your appointment details if you have them
• You have to have seen a GP at the Surgery about the problems / symptoms you are being referred for in order to facilitate the referral
• Check with your insurer that the provider and the treatment you are seeking are covered on your policy

Such requests take up to 10 working days to turnaround
